Middle School Sports Information

All students who wish to try out for a athletic team must have a valid physical and a completed emergency medical form in Final Forms. Please log in to your Final Forms account to verify your student’s information is up-to-date prior to the start date of the sport. Head coaches from each sport will share additional information when it becomes available. Contact Middle School Athletic Director Michael Summers with any questions.

Sports Physicals

State certified sports physicals are available through your families healthcare provider, area urgent cares and CVS Minute Clinics. Please check with each service provider on their insurance coverage policies prior to your appointment.

Required Physicals

Interested in playing?

1. All athletes must first register at shaker-oh.finalforms.com prior to participation. Complete and sign (parents/guardians and athletes) each form.

 

2.  Upon completion, print the OHSAA Physical form from the FinalForms website. While the first three pages will be filled in on FinalForms, a physician must complete the form during an actual physical exam. Phyicals are valid for 13 months.

 

3. Submit the completed physical form to the Middle School main office before your season begins.

2023 Fall Athletics Information

 

All parents of student-athletes in Grades 7-12 must sign-up for FinalForms to receive communications from the Athletic Department about completing pre-participation forms, team emails, and other updates. To register your student athlete login and create an account at https://shaker-oh.finalforms.com.

 

Completed physicals should be submitted to your school building's Main Office. Physicals are good for one calendar year. School staff will enter the expiration date and FinalForms will automatically send you an alert email when the physical expiration date is under 60 days.

 

Cross Country (Boys & Girls)

Start date: Monday, August 14 from 4-5:30 p.m. at Shaker Middle School (Outside Gym Doors). Everyone makes the team that meets OHSAA eligibility requirements.

Girls Tennis   

Coach: Mary Jane Miller (mjm_pace@yahoo.com)

Tryout Date: Tuesday, August 8, 6-7 p.m. at SHHS courts (NEW DATE). Players should already know how to hit a forehand, backhand, and serve. Players will need their own racquet and appropriate tennis shoes as well. If this date is  rained out, the backup date is Tuesday, August 8, 4-5 p.m.

Volleyball
