Important: How to Report a COVID-19 Case During the Winter Recess

December 20, 2021–Thank you to our families, students and staff for continuing to observe safety measures in the community to help prevent the spread of COVID-19, including masking, social distancing and regular hand-washing. The District also continues to urge all eligible individuals to get vaccinated against COVID-19 and receive a booster once eligible.
 
During the winter recess, please continue to report COVID-19 cases among students and staff to the District. In lieu of reporting cases directly to your school building or, for staff, by calling Human Resources, please report cases using this Winter Recess COVID-19 Case Reporting Google Form.
 
By reporting and including all relevant information in your Google Form submission, your case will be recorded and will start the contact tracing and notification process. District staff will follow up with you as needed. Please note, due to the winter recess, the contact tracing and notification timelines may be delayed.
