Student registration forms may be requested by submitting your information below or by downloading the forms at right. You may also call the Registration Office at (216) 295-4321.

Please bring your completed forms with you when you appear in person at the Registration Office located in the Administration Building, 15600 Parkland Drive. Office hours are from 8 a.m. to 4:30 p.m. Monday through Friday.
