Registration Materials

Student registration forms may be requested by submitting your information below or by downloading the forms at right. You may also call the Registration Office at (216) 295-4321. 

Please schedule an appointment to bring your completed forms to the Registration Office located in the Administration Building, 15600 Parkland Drive. Office hours are from 8 a.m. to 4:30 p.m. Monday through Friday. You may schedule an appointment by calling (216) 295-4321.
