Student registration forms may be requested by calling the Registration Office at 216-295-4321 or downloaded below. The forms are interactive and may be completed online, then printed. Please bring your completed forms with you when you appear in person at the Registration Office located in the Administration Building, 15600 Parkland Drive. Office hours are from 8 a.m. to 4:30 p.m. Monday through Friday. For the list of original documents needed when completing the registration process, please visit the Registration Information page.
