Your child may not enter school without verification of the following immunizations as required by Ohio law (revised in 1999):
a. five doses of DPT (diphtheria, pertussis, tetanus) vaccine,
b. four doses of OPV/IPV (poliomyelitis) vaccine,
c. two doses of MMR (measles, mumps, rubella) given on or after the first birthday,
d. three doses of HBV (hepatitis B)
District policy requires that all parents/guardians complete a Tuberculosis Risk Questionnaire to determine whether the child needs a tuberculosis (TB) test prior to entering school.
Included in the registration packet are two health forms (green dental form and yellow physical form) that you will need your child’s dentist and physician to complete and sign. These reports must reflect exams conducted within one year prior to registration. You are not required to submit these forms in order to have your child registered for kindergarten. However, these forms are very helpful to school personnel in insuring your child’s well-being and we strongly encourage parents to submit them at the time of registration or shortly thereafter.
Immunizations are available from your family doctor or, for an affordable fee, from the Shaker Heights Health Department by appointment at 491-1480.
